The Confidential Donor QuestionnaireEvery time a donor attends a donation session they are required to read information about blood safety and fill in a detailed confidential questionnaire about their health and lifestyle. The Western Province Blood Transfusion Service considers this questionnaire to be one of its most important safety precautions that help safeguard its blood supply. It is essential that all donors are unconditionally honest as one inaccurate or false answer could jeopardise the lives of a number of recipients. Once the questionnaire has been completed, donors are confidentially questioned and counselled by clinic personnel which, combined with a physical examination, prevents those individuals at risk of HIV, Hepatitis or other infections from donating. The questionnaire is essential to ensure that donors who may be in the "window period" of HIV infection (the period that the virus cannot be detected by screening tests) do not donate.
